If you or a loved one needs care for a chronic condition, ICPR is here for you. We’ll start by evaluating the patient’s health condition and then work to create a comprehensive care plan. Once that plan is implemented, we’ll monitor and adjust based on physical, mental, cognitive, psychosocial, functional, and environmental factors as well resources and support systems and other providers in our community. Patients and caregivers will be given a copy of the plan so everyone can stay informed.

A comprehensive care plan typically includes, but isn’t limited to:

  • Expected outcome and prognosis
  • Measurable treatment goals
  • Cognitive and functional assessment
  • Symptom management
  • Planned interventions
  • Medication management
  • Environmental evaluation
  • Caregiver assessment
  • Interaction and coordination with outside resources, practitioners, and providers
  • Requirements for periodic review
  • When applicable, revision of the care plan
